The Biographical Information and your picture will be included in the field guide for the trip, and will become part of our MRS Alumni Directory.
The Travel Arrangements form provides us with your hotel preferences, special needs and shirt size for the polo shirts we wear on the first day of the tour.
After you complete and submit this form, you will be prompted to print, fill-out and return the Health Form via campus mail or fax. You may also turn-in the health form at the Orientation. We don't have an online version of this form as it contains your personal health information.
